Herzog & de Meuron wins contest for Danish forest hospital

News: Herzog & de Meuron has won a competition to design a hospital in a Danish forest, with plans for a building shaped like a four-leaf clover (+ slideshow).

Located north of Copenhagen in Hillerød, the New North Zealand Hospital will be Herzog & de Meuron‘s first project in Scandinavia and will be completed in collaboration with local firm Vilhelm Lauritzen Architects.

The building is conceived as a low-rise pavilion-like structure that never exceeds four storeys in height. A total of 24 medical departments will be housed inside and a large garden will be located on the roof.

Herzog & de Meuron says the structure will demonstrate that architectural ambition and functionality can be combined within a hospital.

“The choice of the jury is a seminal sign to architects and the entire health-care sector: low, flat hospital buildings can be better integrated in the city or the countryside than the high-rises structures that were often realised in the last decades,” said the studio.

“The hospital organically reaches out into the wide landscape. Simultaneously its soft, flowing form binds the many components of the hospital. It is a low building that fosters exchange between staff and patients, and it has a human scale despite its very large size.”

The building is scheduled to open in 2020, but could also facilitate an expansion in 2050.

Norwegian medical training facility designed “not to look like a hospital”

Movie: in our next exclusive interview from Inside Festival, Per Anders Borgen of Ratio Architects explains how the design team used raw materials to “remove the institutional look” from the interior of a student facility at St. Olav’s Hospital in Trondheim, Norway.

The Knowledge Centre by Norwegian studios Ratio Architects and Nordic Office of Architecture is a medical student research, training and teaching facility at St. Olav’s Hospital in Trondheim. It won the health category at last month’s Inside Festival.

The outside of the building features a black and white glass facade, designed together with artist Anne Aanerud, which provides shade from the sun as well as decoration.

“Architecture and sunshading form the facade and the expression of the building,” Borgen explains. “That is connected to a very high demand on energy reduction.”

Inside, the architects chose to leave much of the building’s wood and concrete structure exposed.

“Because this is very much a university building, we tried to keep it a little bit rough,” Borgen says. “In hospitals you [usually] have all these clinical, sterile materials. We tried to avoid that.”

“We wanted to use natural wood and concrete. The construction is the interior and that’s part of our concept.”

Patient wards and visitor areas, as well as the student library and cafeteria, feature specially commissioned art works painted on the walls.

“We were trying to remove the institutional look,” Borgen explains. “I don’t think a hospital needs to look like a hospital. That is a convention you can challenge as an architect.”

“It’s very hard because you have all kinds of demands that force you to do something. But [the Knowledge Centre] is an attempt to make a good building with function and good form joined together. It’s not different from every other architectural concept or task in that sense.”

Dyson to build cancer centre after his ward redesign improves baby care

News: British inventor James Dyson has donated £4 million towards building a new cancer centre in Bath, UK, based on the principles underlying his successful redesign of a neonatal ward at the same hospital.

Dyson, who lives and works near to the Royal United Hospital, said he hoped the new centre would be able to replicate the success of the Dyson Centre for Neonatal Care.

“Research has shown the incredible effect that a healing environment can have on recovery,” said the inventor, whose best-known products include a bagless vacuum cleaner and a fan heater with no blades.

“This new cancer centre will use cutting edge technology and well considered design to improve the health of its patients.”

By reducing background noise from hospital machinery and increasing natural light, doctors at the hospital found that the condition of sick and premature newborns improved substantially.

“We have been hugely impressed by the outcomes,” Dyson said of the baby unit, which was designed by local architects Feilden Clegg Bradley Studios.

A study funded by the James Dyson Foundation, the charitable body set up by the designer, found that 90% of babies recuperating in the unit went home breastfeeding, compared to 64% in the old building.

Large windows and skylights increased natural light by up to 50% and exposed babies to changing outside conditions, helping them gain awareness of day and night.

Noise levels were decreased by over 9 decibels on average, helping babies to sleep on average for 22% longer than in the old unit, while nurses in the new building spent 20% more of their time with the newborns.

The cot rooms are arranged in a clockwise circuit from intensive care through to high dependency, special care, the parents’ rooms and finally home, so that parents can clearly track their baby’s route to recovery.

Medical equipment is fixed to the ceiling and pulled down when in use, reducing clutter at ground level.

Dr. Bernie Marden, a consultant neonatologist and paediatrician on the ward, said the study had allowed doctors to build up an accurate picture of how babies respond to their environment.

“We have found that the design of the building is leading to better fed and better rested babies, contributing to their recuperation,” he said.

The Dyson Centre for Neonatal Care is leading the way in improving the quality of life for sick and premature babies. Pioneering research funded by the James Dyson foundation, has found that of babies studied, 90% recuperating in the new unit went home breast feeding, compared to 64% in the old building. The study also showed that babies are better rested – sleeping on average for 22% longer than in the old unit.

Through award winning architecture, the new Neonatal Intensive Care Unit (NICU) creates a healthier environment for babies, parents, and nursing staff. The project was funded in partnership with the NHS and private donations, including £750,000 from the Dyson family and the James Dyson Foundation. The building has a progressive layout. A clockwise circuit of cot rooms, starting with intensive care and leading to special care and finally home. This creates a psychological effect of development. Large windows give controllable natural light throughout, allowing babies, parents and staff to be aware of changing outside conditions, gaining an awareness of day and night.

The Research

A £100,000 donation by the James Dyson Foundation is enabling research to ascertain the full benefits of the new building. Collecting data from both the old building and the new, the hospital is building up a picture of the ideal environment for recuperation.

Consultant neonatologist and paediatrician, Dr. Bernie Marden said: “We have collated vast amounts of data using new techniques to build up a really accurate picture of how babies respond to their environment. We have found that the design of the building is leading to better fed and better rested babies, contributing to their recuperation.”

James Dyson said: “New technology has been specifically adapted to monitor a baby’s sleep cycle and respiratory patterns in a far less invasive way than ever before. The findings show the way in which design and technology can have an effect far beyond the hands of a single consumer – aiding health.”

Accelerometers measure speed and movement; they are used in aircraft and smartphones and increasingly in sports and athletics. Bath Rugby Club uses the technology to analyse player training techniques and fitness.

This research is the first in the world to adapt and use accelerometers to measure the respiratory and sleep patterns in a baby in order to monitor their reaction to the surrounding environment, using an extremely low power, self contained wireless device. Previously intrusive methods including ECG and information from ventilator circuits have been used to measure this.

The accelerometers have been found to be sensitive enough to provide remote and wireless respiratory information. Doing away with invasive tubing and tangled wires. This is a significant result which may allow for remote monitoring of apnoea, effort of breathing and the quality of sleep. The studied babies in the new centre were found to be asleep or in a restful state for longer than in the old building.

Infrared tracking technology was used to pinpoint staff movements in the building and test the efficiency of the design. The study found that nurses in the new building spend 20% more of their time in the clinical rooms, with the babies. Meaning more time spent caring for the babies.

Lux meters were used to take light measurements according to specific times, dates and outside weather conditions. Up to 50% more natural light was measured in the new building. This ensures a more natural circadian rhythm – allowing the babies, parents and staff to perceive the changing day, aiding the babies sleeping and eating habits.

Sound pressure level meter readings were taken and an average level for each hour was documented in decibels. Noise levels in the special care unit have decreased by over 9dB on average from those in the old building. It is suggested that the increased sleep observed in the babies relates to this reduction in background noise.

Cot side diaries used in the research captured the physiological state of the babies in each environment and the interaction of parents with their babies. In addition a qualitative aspect of the project measured how the ‘intervention’ (the building) affected parental and staff experience, taking the form of semi structured psychological interviews.

World Architecture Festival 2012: architects Kristen Whittle and Ron Billard explain how meerkats, fish and open-plan offices for consultants helped provide better care for sick children at the Royal Children’s Hospital, Melbourne, the winner in the health category at the World Architecture Festival.

Billard Leece Partnership and Bates Smart collaborated on the design of the hospital, which replaces an existing facility, and the architects describe their decision to remove private rooms from the wards as “one of the most transformational things” they did with the new building. “By interacting with other professionals, [the doctors] are looking at solving problems in a holistic way,” says Billard.

The pair also describe the challenges of redesigning a “much-loved institution.” The new building centres around a six-storey atrium containing grass-like green spaces, a meerkat enclosure and an aquarium. Billard explains how these devices were intended to create spaces that are “relaxing, not stressful” for children. “Hospitals can be pretty scary places,” he says.

Rooms in the each of the wards offer views over the gardens and courtyards. ”The amount of beds in this hospital is more or less the same as the existing facility, so even though it is a much bigger building, there is only the same amount of beds, says Whittle. “The idea is not to deal with vast quantities of kids, but more become a specialist model for only the acute problems.”

New Neonatal Unit by Feilden Clegg Bradley Studios opens in Bath

The Dyson Centre for Neonatal Care opened its doors on the 23rd July to its first babies. The Royal United Hospital Neonatal Intensive Care Unit (NICU), in Bath, has been transferred from its existing small, cramped facilities into its pioneering new home.

The project, funded as a 50/50 partnership by NHS budgets and fundraising by The Forever Friends Appeal, has resulted in a dramatically different and improved environment in which the RUH can care for the 500 premature and sick babies that it looks after each year. A pioneering holistic and therapeutic approach towards the new building has created a new low carbon unit allowing the staff to practice new methods of care for premature and sick babies. The building consists of a single storey new‐build extension, and the refurbishment of the space occupied by the existing NICU facility. The new‐build element accommodates the clinical, support and reception functions as a discreet but contemporary intervention. The refurbished element comprises staff and parents’ facilities. The two elements are linked by a new ‘umbilicus’ which also provides an access point for emergency vehicles. The new building encloses an external courtyard space which provides both vista and breakout from reception and parents areas. The grouping of the care rooms forms a route around the staff base which is the heart of the unit.

The clockwise circuit of cot rooms forms a diagram of intensity of care, beginning with intensive care, then on to high dependency, then special care, on to the parents’ rooms, then finally home. From parents’ feedback progress along this ‘route’ is very important psychologically – it is important that the ever‐decreasing intensity of care is legible to parents. The consulting examination and treatment spaces are carefully daylit. Parents and staff can now perceive changing external conditions through day and night, increasing well‐being. The heart of the clinical area is generously roof lit providing daylight to all the central spaces within. Sunlight is allowed to enter the building in certain controlled areas to add sparkle and delight without disturbing the working of the unit. Within the care areas light is carefully controlled to ensure that babies gain an awareness of day and night as they develop.

Sustainability in construction and use has been central to the design of the new unit. The team were adamant that the new NICU should not be a one‐off showcase for sustainability, but should serve as a template and catalyst for sustainable healthcare design by challenging existing standards, defining new targets and developing strategies replicable elsewhere in the health sector. The unit is constructed entirely in cross laminated timber. This construction has benefits in terms of embodied energy, and is quick and clean to construct within a healthcare environment. The timber is exposed internally, creating a more calm and domestic environment within an acute clinical setting.

The project has achieved Breeam “Excellent”, and incorporates a sedum roof for rainwater attenuation, and to increase biodiversity on the site. These measures will combine to make the new Neonatal Intensive Care Unit at the Royal United Hospital a beacon for sustainability across the health sector. The building is a case study for the next generation of healthcare buildings, a benchmark for best practice which incorporates replicable strategies which are demonstrated to pay back in a defined period, in a beautiful and therapeutic environment which is of tangible benefit to parents, staff and patients.

The first baby to have experienced both the cramped and noisy conditions of the old NICU and the benefits of the new facilities, Joshua Heather, was transferred to the Dyson Centre for Neonatal Care at the weekend. His mother, Cheralyn, said she is thrilled to be part of this memorable day and is amazed at the light airy conditions of the new building. “It is a real transformation and it will give me the opportunity to spend more time with Joshua in such comfortable and spacious surroundings”.
